I'm writing under the UTAH public records law to acquire the public record documents regarding physician Eric C. Nielson, License Number: 274625-8905 and License Number: 274625-8905.

This would include:

- all medical malpractice case information, including the docket number and county

- all complaints and disciplinary actions

- all hospital admitting privileges

- all correspondence complaints 

- all limited licenses and temporary licenses

This would be for all years since they have been licensed.
