August 20, 2020: Inspection discovered several deficiencies, including improperly stored oxygen canisters, lack of proper documentation on sterilized equipment to ensure proper sterilization, feeding stray cats outside (infection control issue), and failure to provide abortion patient with contact info for hospital nearest her home. -

Inspection report found numerous violations and deficiencies, including: failure to report suspected abuse or neglect or statutory rape or sexual assault when dealing with minors aged 14-16 and even under the age of 14, failure to store hazardous cleaning materials in a secure manner therefore risking harm to patients, failure to properly administer abortion-inducing drugs in the facility under a physician's supervision as required by the FDA for patient safety, failure to implement written discharge instructions with the name and phone number of the nearest hospital for the patient if she has complications or an emergency arising from the abortion.
