Failure to ensure that Administrator and Financial Officer were listed on the clearinghouse roster

Lab supplies, materials, and specimens not properly stored for accurate test results. Collection bins expired. Patient records not maintained nor kept complete, up-to-date, progress noted, or kept confidentially. A patient under anesthesia was not being monitored or checked for vital signs.

Lab and blood work area same as waiting room. Emergency equipment and patient monitoring equipment not maintained. Staff not adequately or properly trained for medical care or emergencies.

Clinic receives warning notice with a $275.75 fine, because they submitted an incomplete abortion clinic license application.
