Dr. Sarah Forbes was fined over $430,700.00 by the U.S. Dept. of Health and Human Services after being audited for fraudulent and deceptive billing practices. She was also disciplined by the Virginia Medical Board for engaging in fraudulent patient billing, falsifying medical records, prescribing drugs without examining patients, knowingly prescribing controlled substances to addicted persons, and falsifying her birthdate on official documents to make herself appear younger. Forbes later surrendered her medical license after failing to complete her clinical competency exams. She passed away on June 24, 2011 at age 85, after rear-ending a tractor while driving her H2 Hummer. It is believed she was distracted by her four pet cocker spaniels which were riding in the car with her.

Amended Order and Findings of Fact - VA Board of Medicine, 1-6-2010
Filed Jan. 6, 2010. The U.S. Dept. of Health and Human Services finds Sarah Forbes guilty of fraudulent patient billing and falsifying records. Forbes is fined a settlement fee of $430,757.36. The VA Board of Medicine also finds Dr. Sarah Forbes guilty of numerous legal violations, including: 1) fraudulent over-billing of patients, 2) falsifying medical records to support false billing charges, 3) failing to examine patients before prescribing drugs, 4) allowing a patient to manage her own medical records, 5) pre-signing blank prescription forms and leaving them in unsecured areas, and 6) falsifying her birthdate on medical license forms to make herself appear younger. In conclusion, the Medical Board announces the following discipline measures for Dr. Forbes: 1) The Board places Forbes on indefinite probation. 2) She is prohibited from prescribing certain controlled substances. 3) She must complete specified amounts of continuing medical education. 4) She must pay a $500 inspection fee. 5) She must certify her correct birthdate of May 4, 1926 and submit it to all affiliated hospitals and federal, state, and local agencies. 6) Her office will be subject to an unannounced inspection. 7) She is required to complete 12 hours of continuing medical education, and a Special Purpose Examination to keep her license. -

Allegations include: 1) fraudulent billing of patients, 2) falsifying medical records, 3) prescribing controlled substances to patients without proper medical exams, 4) pre-signing blank prescription forms and leaving them in unsecured locations, 5) falsifying her birthdate on medical license documents, 6) prescribing narcotics and pain medications without documentation, 7) failing to inform patients of the medical risks of drugs, and 8) knowingly prescribing controlled substances to patients who were addicted to those drugs.
