Multiple violations and deficiencies on basic health and safety regulations. DEFICIENCIES and infection risks for patients.

13 pages of violations including infection control issues, use of contaminated IV supplies, lack of training, sloppy record keeping, etc. Gave one woman Misoprostol to begin her abortion even though her records clearly showed she was over the legal limit. She was discharged and referred to another facility.

Failure to ensure that safe injection practices were followed, that correct hand-washing technique was utilized by staff, and that critical multi-use equipment was sanitized between patients

Multiple violations, hospitalized patient, incomplete abortions requiring additional surgery, drugs checked out and never checked back in, infection control violations, etc.
