Inspection, June 5, 2012 - Report findings include: facility failed to ensure practitioners documented informed consent disclosing comparative risks, benefits, and alternatives of procedures, facility failed to properly evaluate risk of anesthesia before each patient surgery, and facility failed to ensure patients were properly identified by the operating surgeon prior to starting a procedure. PA Health Dept. Inspection, Sept. 29, 2011 - Report found that clinic "failed to maintain a safe and sanitary environment." Findings include: blood stains on windows of laboratory, five cans of soda stored in "Medications Only" refrigerator, and three physicians dispensing Valium when only one was licensed to do so at that facility.
