2018 Investigative Report from the Florida Dept of Health. On Dec 11, 2017, patient "K.N.M" went to Roth's abortion clinic in Hollywood and signed consent forms for a pill abortion. Patient states she was forcibly given a surgical abortion, and inadvertently saw her fetus on the ultrasound. Patient says staff injected her in the arm and she woke up later in a recovery room. She was told that the doctor did a surgical abortion and her chart got mixed up with another patient. She states Dr. Roth never apologized to her, and she was forced to sign a consent form for a surgical abortion after-the-fact. When she requested her medical records a few days later, clinic staff claimed her original consent forms for the pill abortion were "accidentally shredded." K.N.M. filed a complaint, in which she alleged Dr. Roth provided substandard care, performed a wrong/unauthorized procedure, and failed to maintain records.
